SUMMARY OF DUTIES

Providing Parents / Caregiver(s) with the skills to maintain a safe and nurturing home for the family. This is accomplished by a “hands-on” approach, addressing issues of: cleanliness & organization of the home, identifying any safety hazards in the home, educating on nutrition, meal preparation & storage, budgeting etc. Services are typically provided in the home.

PRIMARY DUTIES

Depending on specific program assignment, may perform any or all of the following:

  • Provide instruction on essential life skills such as cooking, cleaning, budgeting, and household safety.
  • Assess Parent / caregiver / family’s needs and interests and provide supportive services / resources as needed while regularly updating IHOC / Case Manager.
  • Engage family in activities that promote togetherness and team work such as games, craft activities, home decorating projects.
  • Maintain accurate and current progress notes on all contracts, outcomes and follow up plans. Progress notes and in-home visits forms must be submitted within 24 hours of the visit or contact.
  • Maintain client files, reports, and statistics per agency and funding requirements.
  • Works with SHIELDS staff to develop program objectives, implementation strategies, and evaluation criteria to refine and enhance service delivery.
  • Maintain and develop working relationships with local linkage and referral agencies.
  • Mandated Reporting – T&D Instructors are mandated reporters. If you suspect or observe abuse of any kind, contact your programs Clinical Supervisor or a Program Manager immediately to inform and consult for next steps.
  • Maintain caseload of 10 – 15.
  • Assist residents with housing resources (i.e., permanent, transitional, low income).

Requirements:

These specifications are general guidelines based upon the minimum, ordinarily considered essential, to satisfactory performance in this position. Individual skills and abilities may result in some deviation from these guidelines.

To perform effectively in this position, the employee must me the following requirements:

  • Be proficient in reading, writing in Spanish (preferred).
  • Possess a working knowledge of conducting interventions for residents with a history of mental illness.
  • Develop and maintain a strong working knowledge of facility operations and procedures.
  • Know and comply with Agency policies and procedures, HIPAA, Department of Mental Health, policies and documentation guidelines, and other state/federal regulations relating to service programs for adults with a history of mental illness.
  • Present ideas, information, and viewpoints clearly, both verbally and in writing.Efficiently use the personal computer to include a variety or word processing software.
  • Utilize analysis, experience, and judgment to make decisions.
  • Demonstrate commitment to team objectives and SHIELDS For Families philosophies.
  • Adapt to changing needs by acquiring new skills and knowledge.
  • Current California driver’s license and a driving record acceptable to the Agency’s insurance carrier.
  • Provides services within a culturally sensitive model of practice and is respectful to the community and clients.
  • These requirements are typically met by a combination of education at a preferred AA level and experience that includes two years experience working with individuals with a history of mental illness in a social service/mental health environment.
  • COVID-19 vaccination required. May request medical or religious exemption if applicable.
