  • This is a full time Work at Home opportunity for the right candidate.POSITION SUMMARY: Neonatal is a new venture from Healthagen, a business incubator! that identified exciting new product opportunities in the health care and service spaces. The Neonatal Solutions program is aimed at empowering parents/caregivers of babies requiring intensive care and hospitalization. This is an exciting opportunity to help define and create a ground-breaking solution to improve Neonatal outcomes.
RESPONSIBILITIES:
  • The Neonatal Health Coach utilizes technology tools and a collaborative process of assessment, planning, implementation and evaluation to engage, educate, empower, and enable parents/caregivers of NICU babies to achieve and maintain optimal health status for their baby and themselves.
  • Apply data driven methods of identification of members and be accountable to follow appropriate procedures for identifying, screening and assessing member needs.
  • Conduct individualized clinical assessments of member's health concerns/needs and develop a personalized condition-specific action plan.
  • Screen members to identify opportunities for health se! rvices, modifiable risk factors and educational needs and provide appropriate partner referrals.
  • Intervene to positively affect healthcare outcomes.
  • Promote internal and external communication to enhance effectiveness of medical management services.
  • Reinforce appropriate education and monitoring; provide up-to-date health care information to help facilitate the members understanding of their health condition(s).
  • Assist members to be informed and actively engage with their provider in making the best health care decisions.
  • Evaluate the member's progress in setting and meeting established goals and revise their individualized care plan accordingly.
EDUCATION:
  • Bachelor's degree or equivalent experience.LICENSES:
  • RN License is required.REQUIRED SKILLS:
  • Clinical experience in pediatrics and/or the NICU is required.
  • 3 years of Disease Management experience is preferred.
  • Excellent verbal and written communication skills required to provide patient education.
  • Proficiency in use of computer applications; MS Office experience.
  • A commitment to excellence and accountability.
  • Ability to be flexible and adapt to changing processes.

  • Functional- Nursing/Neonatal Intensive Care
  • Functional- Nursing/Pediatrics
  • Functional- Nursing/Disease Management
